What we know

CareConnect publishes FHIR R4 docs. Certified APIs (ONC 2015 Cures Update): patient access, practitioner access, System Access with bulk export; US Core 6.1.0, SMART App Launch 2.0. Register an application, then request authorization. Also a general-purpose FHIR R4 API. Our generic FHIR mapper (src/connectors/fhir.ts) is written and tested on a synthetic bundle only. It has not been run against this vendor, and it needs the program's level-of-care code map.
